Licensed Practical Nurse (LPN) Skilled Nursing, Carlyle Place, Nights
About The Position
The Licensed Practical Nurse (LPN) provides nursing care and assists with planning and implementing resident care focused on comfort, wellness, and quality outcomes. Under the supervision of a Registered Nurse or licensed practitioner, the LPN participates in assessments, delivers treatments, monitors resident conditions, and contributes to individualized care plans. The position serves residents in Skilled Nursing, Assisted Living, and Memory Care at Carlyle Place, a senior living community in Macon, Georgia. The schedule is full-time, with 12-hour night shifts from 7:00 PM to 7:00 AM and an every-other-weekend rotation. The posting also describes one 6-hour shift each week.
Requirements
- Graduate of an accredited School of Practical Nursing.
- Current, unrestricted Georgia LPN license, or compact license eligible for practice in Georgia.
- Current CPR certification or ability to obtain it within 60 days of hire.
- Basic Life Support (BLS) certification required per policy.

Responsibilities
- Provide compassionate nursing care to residents in Skilled Nursing, Assisted Living, and Memory Care settings.
- Monitor resident conditions and report significant changes to the healthcare team.
- Assist with assessments, care planning, treatments, and procedures.
- Administer medications and treatments within scope of practice.
- Educate residents and families regarding care plans and treatments.
- Document assessments, interventions, and resident responses accurately and timely.
- Collaborate with interdisciplinary team members to support positive resident outcomes.
- Ensure care is delivered safely, efficiently, and in accordance with regulatory requirements.
