About The Position
Nightingale Visiting Nurses provides compassionate, around-the-clock care so patients can maintain independence and dignity in the comfort of their own homes. This role involves providing direct patient care and case management in the home, conducting patient admissions, developing individualized care plans, obtaining medical history and performing brief physical exams, delivering skilled nursing care in compliance with HIPAA, evaluating the effectiveness of nursing services, and coordinating total patient care. You will also be responsible for submitting patient notes and documentation in a timely manner.
Requirements
- Licensed LPN in Massachusetts
- Physical exam within 12 months prior to first patient contact
- TB test within 12 months
- CPR certification
- Valid driver’s license
- Ability to attend 2-day orientation at Taunton office
Responsibilities
- Provide direct patient care and case management in the home
- Conduct patient admissions and develop individualized care plans
- Obtain medical history and perform brief physical exams (vitals, mental status, diet, etc.)
- Deliver skilled nursing care in compliance with HIPAA
- Evaluate effectiveness of nursing services and coordinate total patient care
- Submit patient notes and documentation in a timely manner
