About The Position

The Licensed Practical Nurse (LPN) performs nursing functions within the scope of practice and provides and/or coordinates care and services to meet residents’ physical, psychosocial, and functional care needs. The role works collaboratively with the interdisciplinary team to develop individualized plans of care, supports regulatory compliance, and promotes quality clinical care through teaching and role modeling professional nursing practices. The LPN administers medications and treatments, may initiate IV therapy when IV-certified, monitors residents’ status, and completes clinical documentation using the electronic medical record. The position also coordinates continuity of care, oversees certified staff and others as assigned, maintains a safe environment, and follows protocols for investigating resident or staff incidents and accidents. The role is based at Breeze Park and is listed as full-time, day/evening.

Requirements

LPN licensure in the state of practice.
CPR certification.
Ability to lift 30 pounds and push or pull 50 pounds.
Ability to perform the physical and sensory duties described, including standing, walking, handling objects or controls, sitting, reaching, stooping, kneeling, crouching, crawling, and occasionally climbing or balancing.
Willingness to work beyond normal working hours and on weekends and holidays when necessary.

Nice To Haves

Specialized training or experience in gerontological nursing.
IV certification, if initiating IV therapy.

Responsibilities

Practice nursing within the scope of licensure and applicable state and federal rules, regulations, and EverTrue policies and procedures; maintain an active license.
Collaborate with the interdisciplinary team to define individualized resident care plans and work with nursing leadership to support regulatory compliance.
Promote learning and provide teaching and role modeling to staff to support quality clinical care.
Pass medications and/or oversee medication ordering and administration; administer medications and treatments and monitor ordered IV fluids, supplemental nourishments, and medications.
Initiate IV therapy when IV-certified.
Maintain knowledge of observation, assessment, and documentation processes appropriate to the level of licensure, including care and service plans and the MDS as applicable.
Use the electronic medical record to complete clinical documentation in accordance with policies and procedures.
Observe resident status, coordinate with resident services to ensure continuity of care, and maintain timely, accurate, and detailed medical reports.
Assess potential hazards and implement practices that maintain a safe, comfortable, and functional environment.
Oversee and direct certified staff and others as assigned to ensure EverTrue standards of care and service are met.
Report and support investigative protocols for staff or resident incidents and accidents in accordance with state and federal regulations.
