About The Position
Signature Albany Home Health is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide quality care to patients in the comfort of their homes. This role offers meaningful one-on-one patient care, flexible scheduling, and a supportive team environment. Under the supervision of a Registered Nurse, the LPN provides direct patient care, implements individualized care plans, administers treatments, educates patients and families, and collaborates with the care team to support positive patient outcomes.
Requirements
- Graduate of an accredited Practical Nursing program
- Current Oregon LPN license
- Current CPR certification
- Strong communication and organizational skills
- Valid driver's license, reliable transportation, and current auto insurance
Nice To Haves
- Home health, community health, or medical/surgical experience preferred
Responsibilities
- Provide skilled nursing care in accordance with physician orders and plan of care
- Administer treatments and monitor patient responses
- Document patient care accurately and timely
- Educate patients and caregivers on care plans and disease management
- Communicate changes in patient condition to the RN Case Manager
- Collaborate with the interdisciplinary team to ensure quality patient care
