About The Position

Signature Albany Home Health is seeking a skilled and compassionate RN Case Manager / SOC RN to provide high-quality home health care to patients in the comfort of their homes. This full-time role is ideal for an RN with experience in home health, Start of Care (SOC) assessments, care coordination, and patient-centered care, who thrives in an independent, field-based environment while collaborating with an interdisciplinary team.

Requirements

Graduate of an accredited nursing program.
Active Registered Nurse (RN) license in Oregon.
Minimum 2 years of nursing experience, preferably in home health or public health.
Current CPR certification.
Valid driver's license and reliable, insured vehicle.

Nice To Haves

Experience completing Start of Care (SOC) assessments preferred.

Responsibilities

Perform comprehensive nursing assessments, including SOC visits, and develop individualized plans of care.
Coordinate and manage home health services in collaboration with physicians and care teams.
Provide skilled nursing care, treatments, and patient education.
Monitor patient progress and communicate changes in condition to physicians.
Implement preventive, rehabilitative, and restorative nursing interventions.
Identify and coordinate discharge planning needs.
Maintain accurate and timely clinical documentation and patient records.
Communicate with healthcare providers and community resources.
Ensure access to necessary medical equipment and services.
Supervise and evaluate Home Health Aides (HHAs), CNAs, and LPNs/LVNs.
Provide guidance to ensure quality care delivery and compliance.
Participate in quality assurance and performance improvement programs.
Attend training and in-service education.
Participate in on-call rotation as required.
