About The Position
The Licensed Practical Nurse (LPN) provides compassionate, professional nursing care to residents in a skilled nursing and long-term care environment. The LPN assists in planning, implementing, and evaluating resident care under the supervision of a Registered Nurse (RN) and in accordance with the Indiana Nurse Practice Act, facility policies, and state and federal regulations.
Requirements
- Current LPN license in good standing with the Indiana State Board of Nursing.
- CPR certification (BLS) required.
- Strong clinical, communication, and leadership skills.
- Ability to work collaboratively as part of an interdisciplinary healthcare team.
- Must be able to lift, move, and assist residents as needed.
Nice To Haves
- Prior experience in skilled nursing or long-term care preferred.
Responsibilities
- Provide direct nursing care to residents, including medication administration, treatments, and documentation.
- Monitor residents’ health status, observe for changes in condition, and report promptly to the RN or physician.
- Assist with developing, updating, and implementing individualized care plans.
- Supervise and direct Certified Nursing Assistants (CNAs) to ensure quality resident care.
- Maintain accurate, timely, and complete medical records and documentation in accordance with facility and regulatory requirements.
- Participate in admission, discharge, and transfer processes for residents.
- Communicate effectively with residents, families, physicians, and interdisciplinary team members.
- Promote residents’ rights, dignity, and independence.
- Follow infection control, safety, and emergency procedures.
- Participate in continuing education and staff development activities.
