Licensed Practical Nurse (LPN) – Per Diem Home Health – Downtown Miami
About The Position
Matrix Home Care is seeking experienced and compassionate Licensed Practical Nurses to perform home health visits in the Downtown Miami area. This role is critical in initiating patient care, completing comprehensive assessments, and ensuring accurate OASIS documentation for home health patients. The LPN provides home-based care to patients with illnesses, injuries, and chronic conditions, often following discharge from a hospital, rehabilitation center, or nursing facility. The role includes skilled nursing services, case management, patient and family education, care coordination, monitoring patient progress, and supporting appropriate discharge from services. The LPN also collaborates with other care providers and community health resources to help ensure high-quality patient care. This is a per-diem position with competitive per-visit rates and flexible scheduling. The position is subject to Florida background screening, including fingerprinting and eligibility review, before employment.
Requirements
- Active Florida Licensed Practical Nurse (LPN) license in good standing.
- Minimum 1 year of home health experience.
- Strong experience with OASIS documentation.
- Excellent clinical assessment and critical thinking skills.
- Ability to work independently and manage a flexible schedule.
- Reliable transportation and a valid driver’s license.
- Completion of background screening under Chapter 435, Florida Statutes, including fingerprinting and review for eligibility before employment.

Responsibilities
- Examine patients, complete clinical assessments, document progress, and report changes to physicians.
- Complete accurate and timely clinical documentation, including OASIS, evaluations, physician orders, and care coordination notes.
- Develop, implement, and continuously monitor patient care plans in collaboration with patients and their representatives.
- Deliver skilled nursing services and provide case management for assigned patients.
- Educate patients on their plan of care, medications, therapies, diet, and exercise.
- Coach and support family members on patient care, medication management, and home safety.
- Coordinate care with physicians, discharge planners, and interdisciplinary teams.
- Oversee and collaborate with LPNs, CNAs, and home health aides providing patient care.
- Monitor patient progress, outcomes, and changing needs, adjusting care plans accordingly.
- Ensure care goals are met and coordinate appropriate discharge from services.
- Serve as a liaison with community health resources, including DME providers, oxygen services, and infusion therapy providers.
- Maintain ongoing communication with patients, families, physicians, clinical managers, and office staff.
- Participate in interdisciplinary team meetings and staff conferences.
- Engage in continuous education and professional development opportunities.
- Develop and follow work methods and procedures that support high-quality patient care.
