About The Position
Brookestone Home Health & Hospice is seeking a Full-Time Licensed Practical Nurse (LPN) to provide quality, patient-centered care through our Home Health and Hospice services in Grand Island and the surrounding communities. In this role, you'll have the opportunity to care for patients in the comfort of their own homes while supporting their health, independence, dignity, and overall well-being. As a member of our care team, you will work closely with patients, families, physicians, and interdisciplinary team members to deliver individualized care plans and ensure exceptional outcomes. Whether assisting patients with skilled nursing needs or providing comfort and support during the hospice journey, you'll play an important role in enhancing the lives of those you serve.
Requirements
- Hold a current, active Licensed Practical Nurse (LPN) license
- Maintain current CPR certification
- Demonstrate strong personal integrity and professionalism
- Possess excellent communication and interpersonal skills
- Exhibit a compassionate, patient-focused approach to care
- Thrive in a collaborative team environment
