About The Position

Cornerstone Family Healthcare is actively recruiting for a Part Time LPN to join our Internal Medicine team in Middletown, NY. This role involves performing screening procedures, documenting patient information, identifying potential patient problems, and alerting the RN or Provider. The LPN will assist providers with in-office procedures and exams, participate in the Care Team Model including Pre-Visit Planning and care team huddles, and direct population management activities. Responsibilities include administering medication, treatments, and vaccines, performing hearing and vision screenings, carrying out provider orders for EKGs, emergency response, and medication administration. The LPN must adhere to safe nursing practices, including Infection Control, National Patient Safety Goals, Medication Management, and Waived Testing, in compliance with Joint Commission and OSHA. The role also involves referring phone calls and patient triage, floating to other departments as needed, making recommendations for healthcare improvements, ensuring a clean and safe patient environment, and adhering to all clinical, administrative, and general policies. Attendance at required meetings and maintenance of confidentiality are also key aspects of this position.

Requirements

  • Current NYS Licensed Practical Nurse (LPN)
  • Diploma from an accredited LPN program/school
  • Current BLS/CPR certification

Responsibilities

  • Perform screening procedures and documentation in accordance with CORNERSTONE policies and procedures.
  • Identify potential patient problems, alert the RN or Provider and anticipate orders for intervention.
  • Perform screening procedures according to CORNERSTONE policies.
  • Assist providers with in-office procedures and exams as needed.
  • Participates in CORNERSTONE’s Care Team Model including but not limited to Participates in Pre-Visit Planning (see Pre-Visit Planning Policy).
  • Leads care team huddles/communication.
  • Directs population management activities for assigned patient panel.
  • Directs and participates in quality improvement activities within the care team and department.
  • Administer medication, treatments and vaccines according to provider orders and scope of practice.
  • Perform hearing and vision screenings on children and adults.
  • Responsible for carrying out Medical Providers orders for the following actions: EKG’s, Emergency response, medication administration (Oral Medications, vaccinations, prescribed treatments) and other procedures according to Provider orders.
  • Strictly adhere to safe nursing practice including but not limited to Infection Control, National Patient Safety Goals, Medication Management and Waived Testing set forth by CORNERSTONE policy and incompliance with Joint Commission and OSHA.
  • Refer phone calls and patient triage to Lead Nurse or Physician.
  • Float to other departments and the laboratory as needed.
  • Make recommendations to supervisors for the improvement of health care to all patients.
  • Ensure clean and safe patient environment.
  • Be familiar with and adhere to all clinical, administrative, and general policies and procedures as outlined in the CORNERSTONE Policy and Procedure Manual and Employee Handbook.
  • Attend and participate in monthly department, All Staff and other required meetings.
  • Maintain confidentiality of all aspects of CORNERSTONE including, but not limited to, patient confidentiality, financials, and employee relations.
