About The Position

Join The Care Team as a Hospice LPN, providing compassionate, hands-on care and support to patients and families during end-of-life care. The role delivers high-quality patient care, supports the interdisciplinary team, and helps ensure patient comfort and dignity in home and facility settings. The LPN provides nursing care as delegated by the RN and contributes to symptom management, patient monitoring, and care documentation.

The standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM. The field territory extends up to a one-hour radius of Mishawaka, Indiana.

Requirements

• Active Licensed Practical Nurse (LPN) license.
• Active driver’s license.
• Ability to pass a criminal background check.
• Ability to pass a Motor Vehicle Record (MVR) check.
• Current TB testing in accordance with agency policy.
• Current CPR certification as required for the role.

Nice To Haves

• Hospice or long-term care experience preferred.

Responsibilities

• Provide nursing care as delegated by the RN.
• Monitor patient status and report changes.
• Support comfort and symptom management.
• Document care provided in the EMR.
• Collaborate with the interdisciplinary team.
