About The Position

Join The Care Team as a Hospice Licensed Practical Nurse (LPN), providing compassionate, hands-on care and support to patients and families during important moments. The role delivers high-quality patient care, supports the interdisciplinary team, and helps ensure comfort, dignity, and exceptional end-of-life care in home and facility settings. The territory extends up to a one-hour radius of Mishawaka, Indiana. Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM.

Requirements

• Active Licensed Practical Nurse (LPN) license.
• Active driver’s license.
• Ability to pass a criminal background check.
• Ability to pass a Motor Vehicle Record (MVR) check.
• Current TB testing in accordance with agency policy.
• Current CPR certification as required for the role.

Nice To Haves

• Hospice or long-term care experience preferred.

Responsibilities

• Provide nursing care as delegated by the RN.
• Monitor patient status and report changes.
• Support comfort and symptom management.
• Document care provided in the EMR.
• Collaborate with the interdisciplinary team.
