Hospice Licensed Practical Nurse (LPN)
About The Position
Grace Hospice is looking for a compassionate Licensed Practical Nurse (LPN) to join their interdisciplinary care team. As a Hospice LPN, the role involves providing hands-on nursing care focused on comfort, dignity, and quality of life. The LPN will work closely with the RN Case Manager and other hospice team members to manage symptoms, provide treatments, educate families, and respond to patient needs in their homes. Requirements
- Current Minnesota LPN license.
- Graduate of an accredited practical nursing program.
- Strong assessment, communication and critical-thinking skills.
- Compassionate and respectful approach to patient and family care.
- Ability to work independently while collaborating with the interdisciplinary team.
- Comfortable using electronic health record systems.
- Valid Minnesota driver’s license, auto insurance and reliable transportation.
Nice To Haves
- Previous nursing experience preferred; hospice or home health experience is a plus.
Responsibilities
- Provide direct nursing care based on each patient’s individualized plan of care.
- Assess comfort, safety and changing needs and communicate updates to the RN Case Manager.
- Support the management of acute and chronic end-of-life symptoms and medications.
- Provide wound care, personal care and assistance with feeding as needed.
- Educate and support patients, families, caregivers and facility partners.
- Collaborate with the interdisciplinary team to provide coordinated, responsive care.
- Participate in bi-weekly interdisciplinary group (IDG) meetings.
- Complete required documentation and follow patient safety, infection prevention, confidentiality and patient-rights practices.
