About The Position

The Licensed Practical Nurse (LPN) provides direct, quality clinical care to residents and supervises nursing assistants working with residents assigned to each unit. The role includes observing and evaluating residents’ physical and emotional status, responding to changes in condition and emergencies, providing care within the scope of an LPN license, helping ensure medication and supply availability, and completing necessary administrative documentation.

This position participates in the Weekend Package Program, working a minimum of two 8-hour or 12-hour shifts every weekend. An additional $5.00 per hour applies for work between 6 p.m. Friday and 7 a.m. Monday; additional shift differentials apply to evening or night shifts on weekends. The position is located at WoodBridge Health Campus in Logansport, Indiana.

Requirements

• Must have and maintain a current, valid state LPN license.
• Current, valid CPR certification is required.

Responsibilities

• Visit residents on the assigned unit daily to observe and evaluate their physical and emotional status, and notify the charge nurse, physician, and/or Director of Nursing of changes in a resident’s condition.
• Identify resident problems and emergency situations and initiate immediate lifesaving measures when a physician is absent.
• Provide holistic resident care within the scope of the LPN license.
• Assist with monitoring the inventory of medications, medical supplies, and equipment to ensure adequate supplies are available.
• Complete administrative duties as necessary, including medical forms, reports, evaluations, studies, and charting.
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals according to facility policies and procedures.
• Work a minimum of two 8-hour or 12-hour shifts every weekend as part of the Weekend Package Program.
