About The Position
Interim HealthCare of Montgomery County is seeking a dependable and compassionate Licensed Practical Nurse (LPN) to provide one-on-one care in a home setting. This role is ideal for nurses who prefer consistent patient assignments, lower patient ratios, and the ability to build meaningful relationships while delivering skilled nursing care. We focus on delivering personalized, high-quality care in the home. Our private duty nursing program allows clinicians to work in a low-stress, patient-centered environment where they can truly make a difference.
Requirements
- Active LPN license in Tennessee
- Current CPR certification
- Reliable transportation and ability to travel to assigned case
Nice To Haves
- Experience in home health, private duty, or long-term care preferred
- Pediatric or adult ventilator/trach experience
- G-tube and enteral feeding management
- Wound care and chronic condition management
Responsibilities
- Provide direct skilled nursing care in the patient’s home
- Monitor and document vital signs, patient status, and changes in condition
- Administer medications and treatments as ordered
- Assist with ADLs (as appropriate to care plan)
- Perform trach care, G-tube care, wound care, or other skilled tasks as needed
- Communicate effectively with RN supervisors, physicians, and care team
- Maintain accurate, timely documentation in the EMR
