Part Time Licensed Practical Nurse (LPN) or Registered Nurse (RN) - Home Care RAI Visits
About The Position
Join BAYADA/Integrity Home Care as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) and work one-on-one with patients in their homes and communities to help them maintain their independence. This role involves conducting annual Resident Assessment Instrument (RAI) and Global Health Evaluation (GHE) assessments for Medicaid clients receiving in-home services. Your assessments will help identify client needs, determine eligibility for services, and ensure they receive appropriate care. You will collaborate with clients, caregivers, and teams to document critical information and support the reauthorization of in-home services. This is a part-time position focused on conducting 10-15 monthly RAI visits in Jasper and surrounding counties.
Requirements
- Current MO RN or LPN license in good standing
- Current CPR certification
- Strong organizational and communication skills
Responsibilities
- Conducting annual RAI and GHE assessments with Medicaid clients receiving in-home services
- Evaluating functional abilities, health status, and care needs
- Accurately documenting findings and maintaining compliance with state requirements
- Collaborating with clients, caregivers, and interdisciplinary teams
- Supporting the reauthorization process for Medicaid-funded services
- Monitoring changes in client status and identifying potential care needs
