About The Position

Requirements

• Licensed as a Practical Nurse / LPN in the state of Minnesota
• Driving is required; must have a reliable vehicle with current insurance and a valid Driver's License
• Demonstrated ability to work autonomously
• Prior Nursing experience in a TCU, Hospice, or Home Health setting
• Military Occupational Classification (MOC) Codes: If you're a military member transitioning to civilian life, we value your service and skills. Applicable MOS codes for this role include: Army: Practical Nursing Specialist (MOS 68C), Navy: Hospital Corpsman (HM), Air Force: Practical Nursing Specialist (4N0X1), Marines: Hospital Corpsman (HM), Coast Guard: Health Services Technician (HS). Nice To Haves

• Home care experience a huge benefit but not required
• Experience with Electronic Medical Records (EMR) software is preferred

Responsibilities

• Provide patient care according to client’s care plan and needs
• Perform wound care treatments and assist clients with medication management
• Teach the client and/or family members about care and support skills
• Communicate regularly with RN Case Managers, Supervisors, and the Care Team
