We are currently recruiting for an RN or LPN to reliably serve patients in the following areas: Jasper + surrounding counties Flexible scheduling - Part Time Visits [10-15 Monthly RAI Visits] No Nights, No Weekends, No Holidays, No On-Call Mileage paid Pay Per Visit [PPV]
Requirements
- Current MO RN or LPN license in good standing.
- Current CPR certification
- Strong organizational and communication skills
Responsibilities
- Conducting annual RAI and GHE assessments with Medicaid clients receiving in-home services
- Evaluating functional abilities, health status, and care needs
- Accurately documenting findings and maintaining compliance with state requirements
- Collaborating with clients, caregivers, and interdisciplinary teams
- Supporting the reauthorization process for Medicaid-funded services
- Monitoring changes in client status and identifying potential care needs
