About The Position

Neighborhood Health, Hartford HealthCare’s mobile healthcare team, provides mobile clinics and street medicine services across Connecticut. This position requires travel to our home-base hubs in Hartford, New Britain, and Norwich, and traveling with the team in company vehicles to clinic and street medicine locations throughout the state. Within the context of the Nursing Professional Practice Framework, the LPN provides quality and cost-effective direct care for patients/families within a unit, service or other primary area of practice. Collaborates with members of health care team to assist patients in achieving established outcomes of care. Participates in direct patient care in order to meet individual and family health goals by obtaining, documenting and reporting patient data related to established plan of care. Performs therapeutic measures, under the direction of an RN or APP, to assist in achieving established patient care outcomes.

Requirements

  • Current Connecticut Licensed Practical Nurse license required.
  • Valid driver’s License
  • Ability to push/pull/lift 50 lbs.
  • Ability to adapt to various work environments as clinics are supported across various community locations (soup kitchens, food pantries, shelters, etc.)

Nice To Haves

  • Medical/Surgical office experience preferred.

Responsibilities

  • Responsible for driving fleet vehicles to and from mobile events.
  • Assist in transport of mobile equipment bins. Load mobile equipment onto vehicles prior to office departure, offload upon arrival at location, load back onto vehicles at close of event and offload upon return to office.
  • Responsible for restocking mobile equipment inventory
  • Coverage required at multiple satellite locations
  • Perform other related duties as required
  • Collaborates with members of health care team to assist patients in achieving established outcomes of care
  • Participates in direct patient care in order to meet individual and family health goals by obtaining, documenting and reporting patient data related to established plan of care
