About The Position

Navitas Healthcare, LLC is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality direct patient care in a long-term care environment. The LPN will be responsible for medication administration, resident assessments, clinical documentation, and collaboration with the interdisciplinary care team to ensure safe, effective, and resident-centered care.

Requirements

  • Active and unrestricted LPN license (state-specific)
  • Ability to work 12-hour shifts, including flexibility for scheduling needs
  • Strong clinical assessment and documentation skills
  • Experience with electronic medical records
  • Ability to work effectively in a fast-paced, team-oriented environment

Nice To Haves

  • Prior experience in long-term care or skilled nursing facility
  • PCC system experience

Responsibilities

  • Administer prescribed medications and treatments in accordance with care plans
  • Monitor resident health status and promptly report changes to the RN and/or physician
  • Perform routine nursing assessments and document findings accurately
  • Provide basic nursing care including wound care and other treatments within scope of practice
  • Maintain timely and accurate documentation using the electronic health record system (PCC preferred)
  • Assist with admissions, transfers, and discharges of residents
  • Support and guide CNAs in delivering resident care as needed
  • Ensure adherence to infection control protocols and facility safety standards
  • Respond promptly to resident needs, concerns, and emergency situations
  • Collaborate effectively with physicians, RNs, CNAs, and other healthcare team members
