Requirements

• Graduate of an accredited nursing program
• Active LPN/LVN license
• Minimum 1 year of clinical experience
• Ability to travel within the coverage area (up to 50%)
• Ability to lift, push, pull, or support up to 50 pounds
• Valid driver’s license, auto insurance, and reliable transportation

Nice To Haves

• Home health or hospice experience preferred

Responsibilities

• Provide direct patient care based on the Plan of Care and physician orders
• Support patient goals through skilled nursing visits
• Monitor patient condition and report changes to the RN Case Manager and care team
• Educate patients and families on self-care and safety
• Document care, medications, and updates accurately
• Collaborate with the RN Case Manager and clinical team
• Help prevent hospitalizations through proactive care
