About The Position

Requirements
- Active Minnesota Licensed Practical Nurse (LPN) license.
- Reliable transportation with a valid driver's license and current insurance.
- Ability to work independently and manage your daily schedule.
- Strong communication skills and a passion for patient-centered care.
Nice To Haves
- Previous nursing experience in skilled nursing, rehab, TCU, hospice, or home health is preferred.
- Experience as an Army 68C, Navy Hospital Corpsman (HM), Air Force 4N0X1, Marine Corps HM, or Coast Guard Health Services Technician (HS) is highly valued.
Responsibilities
- Provide skilled nursing visits in patients' homes.
- Perform wound care, medication administration, and disease management.
- Reinforce education provided by the RN Case Manager.
- Monitor patient conditions and communicate changes to the care team.
- Collaborate with physicians, therapists, and fellow clinicians.
- Complete accurate and timely documentation.
