About The Position

Requirements
- Active Minnesota Licensed Practical Nurse (LPN) license.
- Reliable transportation, valid driver's license, and current auto insurance.
- Ability to work independently and manage a daily field schedule.
Nice To Haves
- Previous experience in skilled nursing, rehabilitation, TCU, hospice, or home health is helpful but not required.
- Compassionate clinicians who enjoy building relationships with patients.
- Military medical experience is highly valued, including Army 68C, Navy Hospital Corpsman (HM), Air Force 4N0X1, Marine Corps HM, and Coast Guard Health Services Technician (HS).
Responsibilities
- Perform skilled nursing visits in patients' homes.
- Provide wound care, medication management, and ongoing patient assessments.
- Educate patients and caregivers to promote independence.
- Communicate changes in condition with the RN Case Manager and healthcare team.
- Complete accurate and timely documentation.
- Help patients remain safe, healthy, and comfortable at home.
