About The Position

As an experienced LPN, you need the freedom to think critically, build relationships, and provide exceptional care without constant interruptions. Home health offers this by allowing you to work independently, manage your own schedule, and care for patients one-on-one, all while being supported by an experienced clinical team that's only a phone call away. This opportunity allows you to practice with greater autonomy while still having exceptional clinical support. You will care for one patient at a time in the comfort of their home, building long-term relationships instead of brief interactions. Requirements

  • Active Minnesota Licensed Practical Nurse (LPN) license.
  • Strong communication and critical thinking skills.
  • Comfortable working independently in a field-based role.
  • Reliable transportation and valid driver's license.

Nice To Haves

  • Previous experience in skilled nursing, TCU, rehab, hospice, or home health is preferred.
  • Military healthcare experience—including Army 68C, Navy Hospital Corpsman (HM), Air Force 4N0X1, Marine Corps HM, and Coast Guard Health Services Technician (HS)—translates exceptionally well into this role.

Responsibilities

  • Deliver skilled nursing visits throughout the Twin Cities Metro.
  • Perform wound care, medication management, and patient assessments.
  • Educate patients and caregivers on treatments and disease management.
  • Communicate closely with RN Case Managers, physicians, and the interdisciplinary team.
  • Complete accurate documentation using our electronic medical record.
  • Help patients remain independent and avoid unnecessary hospitalizations.
