About The Position

Big Bend Hospice is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Home Care team. If you are passionate about providing exceptional end-of-life care and enjoy building meaningful relationships with patients and families in their homes, we'd love to welcome you to our team. This full-time position offers a Monday–Friday schedule, rotating holiday coverage, and the opportunity to make a lasting difference in the lives of those we serve. Working under the direction of a Registered Nurse, you'll provide compassionate hospice care to patients in their homes while supporting families through every stage of the hospice journey.

Requirements

• Current Florida Licensed Practical Nurse (LPN) License
• Minimum of one year of medical-surgical inpatient nursing experience
• Valid Florida driver's license
• Reliable transportation and current auto insurance
• Strong communication and organizational skills
• Ability to work independently while collaborating with the interdisciplinary team

Nice To Haves

• HOSPICE or critical care experience preferred
• Hospice or Home Health experience
• Experience providing compassionate end-of-life care

Responsibilities

• Deliver skilled, patient-centered hospice nursing care in the home setting
• Monitor symptoms and assist with medication management
• Document patient visits accurately and timely in the electronic medical record
• Communicate patient needs and changes in condition with the interdisciplinary team
• Educate patients and caregivers on medications, equipment, and safety
• Assist with care planning and support the RN Case Manager
• Help ensure patients remain comfortable, safe, and supported in their preferred environment
