About The Position

A bonus of up to $9,000 is available for those who qualify! The salary range for this position is $25.97 - $36.36 per hour, based on relevant years of experience. This is a full-time, overnight position located in Fort Collins, Colorado at our Longview campus, requiring 36 hours per week with 12-hour shifts. Employees working at locations operating 24/7 earn a differential. Shift differentials are also available for evenings, nights ($7 per hour), weekends, and holidays. The salary range reflects SummitStone’s good faith belief and reasonable estimate of possible compensation, which may vary based on applicant’s education, training, skills, and experience. The role is responsible for providing direct nursing services and supervising health-related aspects of care for clients, assisting in coordinating medical care and intervention with other team members and providers. Requirements

  • Licensed Practical Nurse license (pass NCLEX-PN exam) required
  • Valid Colorado driver’s license or ability to obtain within 30 days of hire and a clean driving record required
  • DORA license or ability to obtain within 90 days of hire required
  • Basic Life Support Certification, or ability to obtain within 30 days of hire

Nice To Haves

  • More than two years of nursing experience preferred
  • Experience working in behavioral health and substance use preferred
  • Bilingual/bi-cultural preferred.

Responsibilities

  • Deliver high-quality patient care by providing prescribed medical treatment, providing physical assessments, developing plans of care, and implementing medical support services.
  • Administer medication and treatments as prescribed.
  • Gather, monitor, and record patient symptoms, history, and changes in their conditions.
  • Perform standard care procedures like drawing blood, checking vital signs, or collecting lab samples.
  • Coordinate, consult, and problem-solve medical care and intervention with Interdisciplinary team members and providers.
  • Consistently and independently follow established safety procedures.
  • Report any medical or medication errors by end of shift.
  • Maintain updated knowledge of professional licensure and medications procedures, healthcare regulations, standards, and best practices, by actively participating in professional and educational development.
  • Participate in staff meetings and inter-agency meetings as required.
  • Participate in quality improvement initiatives and performance improvement projects.
  • Perform all medical duties including and not limited to client assessment, actively staffing, work at any site, educating clients, administering medications, taking vital signs, medical procedures, telephone/walk in triage for clients, obtain prior authorizations, medication refill requests, lab work, inventory processes, paperwork, and treatment follow-ups.
  • Document and audit the medical record for all medically necessary client care to ensure continuity of care.
