About The Position

Support the referral, screening, and onboarding process for potential and newly admitted hospice patients. This role assists in reviewing incoming referrals, gathering essential clinical information, and coordinating with physicians, referral sources, patients, and families to facilitate timely hospice admissions. Provides clear explanations of hospice philosophy, services, and the admission process to ensure understanding and promote a smooth and supportive transition to hospice care. Working collaboratively with the interdisciplinary team, this role helps ensure that patients who are appropriate for hospice services are identified, informed, and efficiently connected to care.

Requirements

  • Graduate of an accredited school of practical nursing.
  • Completion of the Agency’s Orientation Program of the Valley Home Care.
  • Current and valid NJ license and CPR certification.
  • Ability to use critical thinking skills to set priorities, effectively problem-solve, and perform work accurately with attention to details, often changing from one task to another w/o loss of efficiency or composure.
  • Ability to handle frequent interruptions including emergencies and adapt to changes in workloads and schedules.
  • Ability to function competently in stressful situations related to patient needs (e.g., working w/ patients w/acute, chronic, and complex disease processes, and those who are dying and work w/ patients of varied age groups.
  • Ability to work cooperatively w/ members of the healthcare delivery team, patients, family members, and co-workers.
  • Effective interpersonal and communication (oral and writing) skills.
  • Current and valid driver`s license, registration, inspection sticker and appropriate level of insurance coverage required, to include collision.
  • Employees must adhere to all procedural guidelines in HR Policy 108 including demonstrating an acceptable driving record for the duration of their employment as long as driving continues to be a responsibility of the job.
