About The Position
Post Rehab is seeking a full-time Licensed Physical Therapist to join our rehab team in a skilled nursing setting. This position focuses on restoring movement, reducing pain, and helping residents return to their highest level of function.
Requirements
- Active Physical Therapist license required
Nice To Haves
- Experience in post-acute or long-term care preferred
Responsibilities
- Perform initial and ongoing assessments
- Design and adjust individualized care plans
- Lead treatment sessions targeting mobility, strength, and coordination
- Track outcomes and communicate updates with the care team
- Educate residents and caregivers on carryover strategies
- Complete timely and accurate documentation
