About The Position
Physical Therapist (PT)- Part Time Cheshire, CT Cheshire Civita Care Center $45–$52/hour | Part-Time Post Rehab is adding a part-time Physical Therapist to its rehab team in Cheshire. This role will support resident rehabilitation and mobility goals. This role offers the opportunity to work with a diverse caseload while making a direct impact on patient independence and quality of life. Requirements
- Active Physical Therapist license
- Interest in geriatric and post-acute care
Nice To Haves
- Prior SNF experience is helpful but not required
Responsibilities
- Complete evaluations and establish functional therapy plans
- Provide hands-on treatment to improve strength, balance, and mobility
- Adjust interventions based on progress and clinical judgment
- Document care and collaborate with the interdisciplinary team
