About The Position

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team. This role is responsible for coordinating and overseeing resident health care services on a 24/7 basis in collaboration with the Director of Wellness. The LPN will provide clinical care, support resident service planning, supervise care staff, and ensure high-quality care for all residents.

Requirements

  • Must be 18 years of age or older
  • Active Licensed Practical Nurse (LPN) license in the state of employment
  • Graduate of an accredited nursing program
  • Ability to work flexible hours, including weekends, holidays, and on-call shifts
  • Ability to lift up to 50 lbs (30 lbs regularly)
  • Current driver’s license, reliable transportation, and auto insurance
  • First Aid certification (or ability to obtain within 90 days)
  • Ability to maintain accurate documentation and follow regulatory standards
  • Strong problem-solving skills and ability to work independently
  • Commitment to providing compassionate, resident-centered care

Nice To Haves

  • Minimum of one (1) year experience in home health, assisted living, or geriatric nursing preferred
  • Strong communication and leadership skills

Responsibilities

  • Provide direct nursing care, including medication administration, treatments, and health monitoring
  • Conduct resident assessments, develop and update individualized service plans
  • Communicate with physicians, families, and designated representatives regarding resident status and changes
  • Respond to health emergencies and provide appropriate care within scope of practice
  • Advocate for residents and ensure their physical, emotional, and social needs are met
  • Maintain compliance with all healthcare regulations (HIPAA, OSHA, state regulations, etc.)
  • Ensure high levels of resident satisfaction and promptly address concerns or complaints
  • Supervise and support care staff to ensure quality service delivery
  • Coordinate services with outside healthcare providers and ancillary services
  • Assist with resident admissions, discharges, and transitions of care
  • Ensure accurate documentation, recordkeeping, and physician order transcription
  • Oversee medication ordering and treatment plans
  • Train and mentor care staff in proper procedures and resident care
  • Identify training needs and support ongoing staff development
  • Promote a culture that supports dignity, independence, and quality of life for residents
  • Maintain accurate resident records and reports
  • Ensure compliance with policies, procedures, and infection control standards
  • Participate in on-call rotation and assist with scheduling needs
