More about our team Peak Rehabilitation Hospital is a 52-bed inpatient acute rehabilitation hospital serving the Raleigh/Durham and Apex communities. Operated in partnership with Lifepoint Health, Duke Health, and WakeMed, our interdisciplinary team is dedicated to helping patients recover, regain independence, and improve their quality of life in a collaborative, patient-centered environment.

Requirements

  • Current state LPN license
  • Graduated from an accredited Licensed Practical/Vocational Nurse program
  • CPR/BLS certification required
  • Strong clinical, assessment, and patient care skills
  • Effective communication and patient education abilities
  • Ability to work collaboratively in an interdisciplinary team environment

Nice To Haves

  • IV Certification strongly preferred or must be obtained within 6 months of hire
  • Minimum six months of Medical/Surgical experience in an acute care setting preferred

Responsibilities

  • Administers medications safely and accurately based on patient condition and age
  • Follows proper medication administration procedures to reduce risk of errors
  • Assists the RN with patient assessments and reassessments in accordance with policy
  • Identifies and implements appropriate rehabilitation nursing interventions
  • Completes timely and accurate Quality Improvement (QI) assessments under RN supervision
  • Delivers patient care based on treatment response and updates care plans as needed
  • Assesses and manages patient pain using appropriate interventions
  • Educates patients and families on treatment plans and recovery expectations
  • Promotes patient safety and helps reduce the risk of medical errors
  • Develops individualized patient and family education plans
