About The Position

SPAARK, Inc. is seeking a Licensed Nurse Case Manager to support the Louisiana Army National Guard State Surgeon’s Office. The Case Manager will track Soldier medical cases, coordinate appointments and follow-up care, maintain military medical records, and support medical readiness requirements from initial identification through return to duty or administrative separation.

Requirements

  • Current, unrestricted Registered Nurse license through the Louisiana State Board of Nursing, or ability to obtain required Louisiana licensure.
  • Experience in nursing case management, medical readiness, care coordination, occupational health, or a related field.
  • Knowledge of medical terminology, clinical documentation, and case-management practices.
  • Strong organizational, communication, and follow-up skills.
  • Ability to manage multiple cases and deadlines.
  • Proficiency with Microsoft Office and web-based medical systems.
  • Ability to maintain confidentiality and handle protected health information.
  • Valid driver’s license and ability to travel as required.
  • Ability to meet DoD background, installation-access, training, and system-access requirements.

Nice To Haves

  • Bachelor of Science in Nursing.
  • Two or more years of case-management or care-coordination experience.
  • Military, Army National Guard, DoD, or federal healthcare experience.
  • Experience with MEDCHART, MEDPROS, MODS, eCase, or similar systems.
  • Familiarity with AR 40-501, AR 40-502, and AR 40-66.
  • Professional case-management certification.

Responsibilities

  • Track and manage assigned Soldier medical cases in military systems of record.
  • Review and action MEDCHART referrals daily.
  • Schedule medical evaluations, profile exams, and follow-up appointments.
  • Request and review medical documentation from Soldiers, units, and providers.
  • Coordinate with Soldiers, Medical Readiness Officers, healthcare providers, and State Surgeon personnel.
  • Update MEDCHART, MEDPROS, MODS, and other authorized systems.
  • Track appointments and follow-up documentation to support Soldier readiness.
  • Review health assessment and laboratory results and provide Soldier education.
  • Identify and track Soldiers requiring IDES, MEB, PEB, profile, or fitness-for-duty actions.
  • Prepare supporting documentation for Line of Duty investigations and medical readiness vouchers.
  • Run pre- and post-event MEDPROS reports.
  • Protect medical records and sensitive information in accordance with HIPAA, Privacy Act, DoD, and Army requirements.
  • Support medical readiness events and approved travel requirements.
