About The Position

Easterseals PORT Health (ESPH) is seeking a Licensed Mental Health Therapist to join their Assertive Community Treatment (ACT) Team. This role is designed for clinicians passionate about serving adults with severe mental health and substance use needs through compassionate, community-based care. As a Therapist on the ACT Team, you will provide intensive, recovery-oriented treatment to adults with severe and persistent mental illness, meeting clients where they live, work, and thrive. You will be an integral part of a multidisciplinary team delivering holistic, wraparound care that promotes stability, dignity, and independence. Requirements

  • Licensure: LCSW/A, LCMHC/A, LMFT/A, LPC (provisional or full NC licensure required)
  • Valid driver’s license
  • Reliable transportation
  • Passion for serving vulnerable adults
  • Strong interpersonal skills
  • Clinical judgment
  • Documentation abilities

Responsibilities

  • Provide individual and group therapy to 4–5 adult clients per day in community settings
  • Develop and maintain individualized treatment plans aligned with ACT best practices
  • Participate in daily multidisciplinary team meetings
  • Collaborate with families, community partners, and referral sources
  • Complete timely and accurate documentation in Welligent EMR
  • Participate in the rotating on-call crisis schedule to maintain ACT model fidelity
  • Uphold ethical standards, trauma-informed practices, and client-centered care
