About The Position

Provide in-person psychotherapy and behavioral health services to older adults and medically complex patients in Skilled Nursing Facilities and Assisted Living communities throughout the Catskills. The role offers full-time or part-time scheduling, clinical autonomy, steady patient referrals, and administrative and operational support from an established multidisciplinary team. Providers collaborate with facility staff and build ongoing therapeutic relationships with residents and their families.

Requirements

• Hold an active and unrestricted New York license as a Clinical Psychologist, LCSW, LMHC, or LMFT.
• Have a master's or doctoral degree in an applicable discipline.
• Provide compassionate, patient-centered care.
• Have strong communication and documentation skills.
• Be comfortable providing services in nursing home and long-term care settings.
• Only fully licensed providers are eligible; limited permits, associate licenses, LMSW, MHC-LP, provisionally licensed therapists, and supervised or pre-licensure status are not accepted.

Nice To Haves

• Experience working with geriatric populations.
• Experience in Skilled Nursing Facilities, rehabilitation centers, long-term care, or assisted living settings.
• Interest in interdisciplinary and integrated healthcare.

Responsibilities

• Provide in-person psychotherapy and behavioral health services in Skilled Nursing Facilities and Assisted Living communities.
• Conduct diagnostic assessments and develop treatment plans.
• Treat depression, anxiety, adjustment disorders, grief, trauma, and other behavioral health concerns common among geriatric populations.
• Maintain timely and accurate clinical documentation.
• Collaborate with interdisciplinary care teams to improve patient outcomes.
• Build therapeutic relationships with residents and their families.
