About The Position

Provide in-person psychotherapy and behavioral health services to older adults and medically complex patients in Skilled Nursing Facilities and Assisted Living communities throughout the Carthage, New York area. The role offers clinical autonomy and flexible full-time or part-time scheduling, with administrative and operational support provided by the organization. Providers receive an established referral stream and work with a multidisciplinary team to support residents’ quality of life.

Requirements

Hold an active, unrestricted New York license as a Clinical Psychologist, LCSW, LMHC, or LMFT.
Have a master's or doctoral degree in an applicable discipline.
Provide compassionate, patient-centered care.
Have strong communication and documentation skills.
Be comfortable providing services in nursing home and long-term care settings.
Only fully licensed providers are eligible; limited permits, associate licenses, LMSW, MHC-LP, provisionally licensed therapists, and supervised or pre-licensure status are not accepted.

Nice To Haves

Experience working with geriatric populations.
Experience in Skilled Nursing Facilities, rehabilitation centers, long-term care, or assisted living settings.

Responsibilities

Provide in-person psychotherapy and behavioral health services in Skilled Nursing Facilities and Assisted Living communities.
Conduct diagnostic assessments and develop treatment plans.
Treat depression, anxiety, adjustment disorders, grief, trauma, and other behavioral health concerns common among geriatric populations.
Maintain timely and accurate clinical documentation.
Collaborate with interdisciplinary care teams to improve patient outcomes.
Build therapeutic relationships with residents and their families.
