As our Licensed Medical Social Worker at Mercy Gilbert Medical Center, you will perform critical social work assessments and interventions for inpatient, outpatient, and Emergency Department patients. You'll lead patient-centered care by developing and implementing treatment plans, facilitating post-acute placements, and coordinating comprehensive resources for complex discharges and vulnerable populations. This role is central to crisis intervention, patient advocacy, and ensuring developmentally appropriate care across all served populations, from mentally ill to chronically ill. Every day you will manage psychosocial care, including crisis intervention, counseling, and referrals for diverse issues like abuse, family challenges, and grief. You'll serve as a patient advocate, educating on self-care, bioethics, and insurance, while actively coordinating community resources to address social determinants of health such as homelessness. You'll also contribute to performance improvement teams, ensuring adherence to federal, state, and regulatory guidelines to enhance financial outcomes and reduce readmissions.
