About The Position

Human Touch Home Health is seeking a compassionate and dedicated Licensed Master Social Worker (LMSW) to join our growing team serving Fredericksburg, Stafford, Spotsylvania, King George, Massaponax, Locust Grove, and surrounding communities. As a Home Health Licensed Master Social Worker, you'll help patients and families navigate life's challenges by connecting them with resources, providing emotional support, assisting with care planning, and helping remove barriers that impact health and well-being. Requirements

• Active Virginia Licensed Master Social Worker (LMSW) License Required
• Master's Degree in Social Work from an Accredited Program
• Valid Driver's License and Reliable Transportation
• Strong Communication, Assessment, and Documentation Skills

• Home Health or Healthcare Experience Preferred but Not Required
• Passion for Helping Patients and Families Overcome Challenges

Responsibilities

• Complete psychosocial assessments for home health patients
• Identify social, emotional, environmental, and financial needs
• Connect patients and families with community resources and support services
• Assist with long-term planning and discharge planning
• Provide counseling, education, and emotional support to patients and caregivers
• Collaborate with physicians and interdisciplinary team members
• Maintain timely and accurate documentation
