About The Position
Human Touch Home Health is seeking a compassionate and dedicated Licensed Master Social Worker (LMSW) to join our growing team serving Blacksburg, Christiansburg, Radford, Pulaski, Floyd, Giles County, Montgomery County, and surrounding New River Valley communities. This is a PRN opportunity that offers flexibility, independence, and the ability to make a meaningful impact on the lives of patients and families. As a Home Health Licensed Master Social Worker, you'll help patients and families navigate life's challenges by identifying barriers to care, connecting them with community resources, providing emotional support, and helping them make informed decisions about their health and well-being. Requirements
- Active Virginia Licensed Master Social Worker (LMSW) License Required
- Master's Degree in Social Work from an Accredited Program
- Valid Driver's License and Reliable Transportation
- Strong Communication, Assessment, and Documentation Skills

Responsibilities
- Complete psychosocial assessments for home health patients
- Identify social, emotional, environmental, and financial needs
- Connect patients and families with community resources and support services
- Assist with long-term care planning and discharge planning
- Provide counseling, education, and emotional support to patients and caregivers
- Collaborate with physicians and interdisciplinary team members
- Maintain timely and accurate documentation
