About The Position

Human Touch Home Health is seeking a compassionate and dedicated Licensed Master Social Worker (LMSW) to join our growing team serving Falls Church, Arlington, Alexandria, Fairfax, McLean, Annandale, Vienna, Springfield, and surrounding Northern Virginia communities. This is a flexible PRN opportunity where you can make a meaningful impact on patients and families while maintaining control of your schedule. As a Home Health Social Worker, you'll help patients and families navigate some of life's most challenging moments. You'll connect individuals with community resources, provide emotional support, assist with long-term planning, and help remove barriers that may impact a patient's health, safety, and quality of life. Your guidance can make a meaningful difference in helping patients remain independent and receive the care and support they need.

Requirements

• Active Virginia Licensed Master Social Worker (LMSW) License Required
• Master's Degree in Social Work from an Accredited Program
• Strong Communication and Interpersonal Skills
• Excellent Organizational and Documentation Skills
• Valid Driver's License and Reliable Transportation

Nice To Haves

• Home Health or Healthcare Experience Preferred

Responsibilities

• Complete psychosocial assessments for home health patients
• Identify social, emotional, financial, and environmental needs
• Connect patients and families with appropriate community resources
• Assist with discharge planning and long-term care planning
• Provide counseling, support, and education to patients and caregivers
• Collaborate with physicians and interdisciplinary team members
• Maintain timely and accurate documentation
