About The Position

The University of Arkansas for Medical Sciences (UAMS) has a unique combination of education, research, and clinical programs that encourages and supports teamwork and diversity. The position is located on the Inpatient Medical Floors and involves shifts Monday-Fridays with rotating weekend days (every 4-6 weeks or so) and rotating holidays. The role involves Psychosocial Assessment/Services, including completing assessments for patients admitted to the hospital, performing discharge planning and aftercare coordination, community referrals and linkages, and providing patient and family education and adjustment to illness counseling, grief counseling and crisis intervention. It also promotes patient and family centered care and documents patient choices in the medical record. Recommends care and services appropriate to the patient's age, developmental stage, social maturity, and cultural background. Communication/Collaboration involves documenting and communicating with the team, physicians, and patient family regarding care coordination. It serves as a consultant for guardianship, power of attorney, health care proxy, and advance directives. Collaborates with other UAMS Departments and participates in hospital-wide performance improvement metrics and regulatory compliance initiatives. Reports and reviews department-specific data metrics monthly and implements, monitors, and evaluates Care Management quality improvement initiatives. Customer Service involves interacting with and assisting the public professionally and friendly, demonstrating effective communication skills, maintaining confidentiality, and fostering positive working relationships. Assist internal and external stakeholders with exceptional customer service. Other duties as assigned to meet UAMS Core Values and Department business needs.

Requirements

  • Masters' degree in Social Work
  • Must be licensed by an Arkansas Professional Board as a Licensed Master Social Worker (LMSW) or Licensed Certified Social Worker (LCSW) by date of hire
  • Proof of legal authority to work in the United States on the first day of employment.

Nice To Haves

  • Certified Case Manager (CCM) or American Case Management (ACM) certification.
  • 2 years experience in an Acute Care Hospital Setting
  • Experience in Case Management

Responsibilities

  • Completes assessment for patients admitted to the hospital.

  • Documents and communicates with all team, physicians, and patient family concerns pertaining to the coordination of care and services.
  • Serves as a consultant for processing guardianship, power of attorney, health care proxy, and advance directives.
  • Collaborates with other UAMS Departments and actively participates in hospital wide performance improvement metrics and regulatory compliance initiatives.
  • Reports and reviews department specific data metrics on a monthly basis.
  • Implements, monitors, and evaluates the Care Management quality improvement initiatives.
  • Interacts with and assists the public in a professional and friendly manner as needed.
  • Demonstrates effective communication skills; communicates accurate and complete information; maintains strict confidentiality when necessary.
  • Demonstrates positive working relationships with co-workers, management team, and ancillary departments; follows the Core Concepts of Patient and Family Centered Care and the Basic Code of Conduct Guidelines.
  • Assist internal and external stakeholders as needed with exceptional customer service.
  • Other duties as assigned in order to meet the UAMS Core Values and the business needs of the Department
