Requirements
- Active Virginia Licensed Master Social Worker (LMSW) license
- Valid driver’s license and reliable transportation
- Strong communication, assessment, and documentation skills
Nice To Haves
- Home health or healthcare experience preferred
Responsibilities
- Assess patients’ social, emotional, and financial needs
- Connect patients and families with community resources and support services
- Provide counseling, crisis intervention, and discharge planning
- Coordinate care with nurses, therapists, physicians, and the interdisciplinary team
- Complete documentation accurately and on time
