Human Touch Home Health is hiring a compassionate Licensed Master Social Worker (LMSW) to provide medical social work services in Blacksburg, Christiansburg, Radford, Pulaski, and surrounding New River Valley communities. This is a flexible PRN opportunity where you can make a meaningful impact by helping patients and families navigate life’s challenges.
Requirements
- Active Virginia Licensed Master Social Worker (LMSW) license
- Valid driver’s license and reliable transportation
- Strong communication, assessment, and documentation skills
Nice To Haves
- Home health or healthcare experience preferred
Responsibilities
- Assess patients’ social, emotional, and financial needs
- Connect patients and families with community resources and support services
- Assist with long-term planning, counseling, and crisis intervention
- Coordinate care with physicians, nurses, therapists, and the interdisciplinary team
- Complete documentation accurately and on time
