About The Position

Navitas Healthcare, LLC is seeking a Licensed Master Social Worker (LMSW) for an exciting Travel or Local job in New York, NY. This role involves providing comprehensive psychosocial assessments, supportive counseling, and discharge planning for inpatient medical and surgical patients. The LMSW will collaborate with a healthcare team to ensure continuity of care and assist patients and families in accessing necessary resources.

Requirements

  • Master’s Degree in Social Work (MSW) from an accredited institution
  • Minimum one (1) year of LMSW experience required
  • Active Licensed Master Social Worker (LMSW) license issued by the New York State Education Department (NYSED)
  • License must be verifiable through Primary Source Verification
  • Strong communication, assessment, and case management skills
  • Experience with psychosocial assessments, supportive counseling, discharge planning, and care coordination
  • Knowledge of HIPAA regulations and patient confidentiality standards

  • Experience in an acute care hospital setting
  • Experience working with inpatient medicine and surgery populations

Responsibilities

  • Conduct comprehensive psychosocial assessments for inpatient medical and surgical patients
  • Provide supportive counseling and therapeutic interventions to patients and families
  • Develop individualized discharge plans to ensure continuity of care following hospitalization
  • Coordinate referrals to community agencies, rehabilitation facilities, behavioral health providers, and social service organizations
  • Collaborate with physicians, nurses, case managers, and other healthcare professionals to address patient needs
  • Assess psychosocial barriers that may impact treatment, recovery, or discharge planning
  • Assist patients and families with accessing healthcare, housing, financial assistance, transportation, and community resources
  • Maintain accurate and timely documentation in accordance with hospital policies and regulatory requirements
  • Participate in interdisciplinary team meetings and patient care conferences
  • Advocate for patients and families to ensure access to appropriate services and support systems
  • Ensure compliance with HIPAA regulations and patient confidentiality standards
