About The Position

Cascade Community Healthcare (CCH) is seeking a Licensed Counselor (Clinician III) or an Associate Licensed Counselor (Clinician II) to join their team. This role is responsible for delivering clinical services to clients, including conducting initial assessments, developing treatment plans, and providing individual, family, and group therapy. The position emphasizes working with a specific program population and requires the ability to collect data and facilitate treatment planning. CCH has been serving Lewis County and surrounding areas for over 50 years with a mission to provide compassionate counseling that leads to positive changes and more satisfying lives.

Requirements

  • Master’s Degree in counseling or the social sciences required.
  • Must be Licensed (LMHC, LMFT, LICSW) or have Associate License credential (LMHCA, LMFTA, or LICSWA).
  • Must be able to pass WA State Patrol background check at hire and annually thereafter. A national background check may be required if have not lived in WA state consecutively the last three years.
  • Must comply with agency policies and procedures.
  • Must maintain current phone number and mailing address on file with the Human Resources office.

Responsibilities

  • Provides intake assessment and treatment planning for assigned clients.
  • Completes intake assessments and treatment plans within timeframe set for by the agency.
  • Reviews treatment plans goals and short-term objectives with client at each individual treatment session, tracks progress towards meetings those goals, and records client responses in client’s own words in the clinical record.
  • Works with client to develop an interim plan for working towards achievement of goals both outside the session and at the next scheduled session.
  • Provides individual, family, group therapy, and case management as appropriate.
  • Meets with client in accordance with expectations outlined in the treatment plan.
  • Ensures proper level of service by reviewing client needs and adjusting treatment plan accordingly to maintain proper utilization of services as evidenced by over/under utilization reports.
  • Meets direct service productivity requirements as assigned by the program manager/director.
  • Coordinates treatment services with schools, other relevant agencies, and significant others.
  • Outlines significance of working with primary care providers and other relevant agencies and significant others at time of development of the treatment plan evidenced by treatment plans and progress notes and releases of information.
  • Periodically encourages client to include the above at various points of treatment when medically necessary or appropriate as evidence in treatment plans and progress notes.
  • Coordinates with the Department of Corrections for Mental Health Sentencing Alternative Program for identified clients.
  • Will respond to phone calls within one (1) business day.
  • Maintains records and reports in accordance with agency policies and procedures.
  • Completes progress notes and data verification to be available to MIS according to agency policy and procedure.
  • Completes treatment plans, crisis plans (as applicable), authorization/reauthorizations of services (if applicable), and progress notes according to agency policy and procedure.
  • Completes other forms as required by agency policy and procedure.
  • Required reports to community partners will be submitted by the 5th of each month.
  • Participates in supervision, consultation, and training at the direction of the Program supervisor.
