About The Position

MPAC Healthcare is seeking Licensed Clinicians (LMHC, LMFT, LCSW) who are passionate about providing high-quality care to seniors. This role offers the autonomy of a private practice without the administrative burden, allowing clinicians to focus on patient care and documentation. You will work as an on-site provider, collaborating with an interdisciplinary team to address the emotional, social, and psychological needs of residents. Requirements

  • Master's degree.
  • Valid LCSW/LMFT/LMHC license in the state for which you are applying.
  • Two years of counseling and therapy experience.
  • Strong medical knowledge.
  • Understanding of documentation (CMS, Medicare, Medicaid).
  • Nursing home/hospital facility experience.
  • Ability to work as a member of a team with a great attitude.
  • Demonstrated knowledge of Mental Health diagnoses and therapeutic approaches.
  • Experience collaborating with interdisciplinary teams.
  • Ability to foster open communication with the facility team and Regional Clinical Manager.
  • Ability to take initiative.
  • High level of organization to manage patient needs and document effectively.

Responsibilities

  • Provide initial assessments.
  • Utilize psychotherapy to address mental health issues.
  • Offer ongoing support to residents and their families.
  • Be responsible for all documentation requirements.
  • Keep up-to-date on Medicare Benefit and commercial insurance plans.
  • Function as an active, lead member on an interdisciplinary team of physicians and nurse practitioners.
  • Manage caseload and schedule.
  • Communicate effectively with patients, families, nursing staff, facility social services, and other MPAC providers.
  • Ensure the mental health needs of residents are met.
