About The Position

This role involves providing comprehensive behavioral health services to individuals with acute care needs. The clinician will conduct assessments, develop treatment plans, lead discharge planning, and provide individual, family, and group therapy. The position requires collaboration with multidisciplinary teams, including psychiatrists, and involves coordinating community services and crisis intervention. The clinician will also manage communication with managed care companies and ensure compliance with regulatory standards. A key aspect of the role is to facilitate a continuum of care, ensuring patients receive maximum benefit both during hospitalization and after discharge, including assisting with necessary equipment and facility placement if needed. The role also supports community relations and requires a commitment to service excellence.

Requirements

  • Ability to read, write and communicate effectively in English.
  • Massachusetts licensure as a Licensed Independent Clinical Social Worker (LICSW) or Licensed Mental Health Counselor (LMHC), or Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT).
  • Crisis Prevention Intervention Training within 1 month of new position.
  • BLS Certification within 3 months of new position and must maintain.

Responsibilities

  • Conducts initial clinical assessments of individuals presenting with acute care needs in accordance with Department of Mental Health’s state requirements.
  • Prepares an Initial Treatment Plan based on assessment findings and communicates it to multidisciplinary teams.
  • Leads treatment and discharge planning in collaboration with the psychiatrist and multidisciplinary treatment team.
  • Conducts group therapy and individual/family counseling and other related group sessions.
  • Assists in patient care planning to ensure continuation of treatment in an atmosphere conducive to preservation of health.
  • Evaluates ongoing medical and psychosocial needs for home care planning to set up appropriate community services.
  • Evaluates data gathered in terms of treatment goals and links the patient to available programs and services for maximum benefit during hospitalization and as a continuum after discharge.
  • Assists patient/family in procuring essential equipment for optimum and safe home function.
  • Assists patient/family in decision-making for selection and placement in an appropriate facility if needed.
  • Brings voluntary community-based services into play as appropriate to patient and family needs.
  • Functions as a liaison on behalf of patients with appropriate community agencies.
  • Evaluates the appropriateness of community services to develop plans to meet patient needs.
  • Initiates crisis prevention and crisis intervention services as needed to newly discharged patients in coordination with the patient's family and primary caregivers.
  • Provides coverage for the Inpatient and Partial Hospital Program Behavioral Health Clinicians.
  • Provides timely communication with managed care companies, offering accurate and complete case accounts to obtain approval for the required level of care.
  • Maintains knowledge of CMS, DPH, DMH, and other regulatory agency standards.
  • Assists with community relations activities, including community contacts and education.
  • Consistently provides service excellence to all patients, family members, visitors, volunteers, and co-workers.
  • Performs other work-related duties as requested/directed.
