The Behavioral Health Clinician will conduct initial clinical assessments for individuals with acute care needs, adhering to Department of Mental Health state requirements. This role involves developing an Initial Treatment Plan, communicating findings to multidisciplinary teams, and leading treatment and discharge planning in collaboration with psychiatrists and the treatment team. The clinician will also conduct group therapy, individual/family counseling, and other related sessions, while assisting in patient care planning for continued treatment and home care needs. Responsibilities include evaluating psychosocial needs, linking patients to community services for in-hospital and post-discharge continuity of care, and assisting patients/families in procuring necessary equipment for home function. The role also involves aiding in decision-making for facility placement if required, utilizing voluntary community-based services, and acting as a liaison with community agencies. The clinician will evaluate the appropriateness of community services, initiate crisis prevention and intervention for discharged patients, and provide coverage for Inpatient and Partial Hospital Program Behavioral Health Clinicians. Additionally, the role requires timely communication with managed care companies, maintaining knowledge of regulatory standards (CMS, DPH, DMH), assisting with community relations, and providing service excellence. The ability to read, write, and communicate effectively in English is essential.
