About The Position

MPAC Healthcare is seeking Licensed Clinicians (LPC, LMFT, LCSW) to join their team and help set a new standard for quality of care for seniors. The Licensed Clinician role allows for autonomous functioning, similar to a private practice, without the administrative burden of managing a back office. The majority of the work week will be dedicated to clinical duties, with the remainder focused on patient documentation and facility correspondence. The clinician will serve as an on-site provider working with residents and their families, acting as a lead member of an interdisciplinary team to address residents' emotional, social, and psychological needs. The role involves providing initial assessments, utilizing psychotherapy, offering ongoing support, and managing documentation requirements, including staying updated on Medicare Benefit and commercial insurance plans.

Requirements

• Master’s degree
• Valid LCSW/LMFT/LPC license in the state for which you are applying.
• Two years of counseling and therapy experience
• Ability to work as a member of a team with a great attitude
• Demonstrated knowledge of Mental Health diagnoses and therapeutic approaches
• Experience collaborating with interdisciplinary teams, including other MPAC providers, to ensure patients are getting the best care possible
• The ability to foster open communication with the facility team and your Regional Clinical Manager to ensure the success of clinical programs
• Ability to take initiative and effectively communicate in multiple settings with multiple parties (patient, patient’s family, nursing staff, facility social services team, other MPAC providers, etc).
• High level of organization to manage patient needs, document effectively and ensure the mental health needs of residents are being met
• Comfortable with the ever-changing environment of a skilled nursing facility
• Knowledge of the Medicare Benefit and commercial insurance plans
• Experience in the healthcare setting
• Passion for working with older adults
• Exceptional organizational skills

Nice To Haves

• Experience in the nursing home space is a plus

Responsibilities

• Function as an active, lead member on an interdisciplinary team of physicians and nurse practitioners addressing residents’ emotional, social, and psychological needs.
