About The Position

MPAC Healthcare is seeking Licensed Clinicians (LPC, LMFT, LCSW) who are passionate about providing high-quality, patient-centered care to seniors. This role offers the autonomy of a private practice without the administrative burden, allowing clinicians to focus on patient care and documentation. You will be an integral part of an interdisciplinary team, addressing the emotional, social, and psychological needs of residents in a skilled nursing facility. Requirements

  • Master's degree.
  • Valid LCSW/LMFT/LPC license in the state for which you are applying.
  • Two years of counseling and therapy experience.
  • Ability to work as a member of a team with a great attitude.
  • Demonstrated knowledge of Mental Health diagnoses and therapeutic approaches.
  • Experience collaborating with interdisciplinary teams, including other MPAC providers, to ensure patients are getting the best care possible.
  • The ability to foster open communication with the facility team and your Regional Clinical Manager to ensure the success of clinical programs.
  • Ability to take initiative.
  • High level of organization to manage patient needs and document effectively.

Responsibilities

  • Provide initial assessments.
  • Utilize psychotherapy to address mental health issues and offer ongoing support.
  • Responsible for all documentation requirements.
  • Keep up-to-date on Medicare Benefit and commercial insurance plans.
  • Function as an active, lead member on an interdisciplinary team of physicians and nurse practitioners addressing residents’ emotional, social, and psychological needs.
  • Manage caseload and schedule.
  • Communicate effectively with patients, families, nursing staff, facility social services teams, and other MPAC providers.
  • Ensure the mental health needs of residents are met.
