About The Position

MPAC Healthcare is seeking Licensed Clinicians (LPC, LMFT, LCSW) to join their team and help set a new standard for quality of care provided to seniors. This role offers the autonomy of a private practice without the administrative burden of managing a back office. Clinicians will function as on-site providers, working with residents and their families, and serving as active members of interdisciplinary teams. The role involves providing initial assessments, utilizing psychotherapy, offering ongoing support, and managing all documentation requirements, including staying up-to-date on Medicare Benefit and commercial insurance plans. Requirements

  • Master’s degree.
  • Valid LCSW/LMFT/LPC license in the state for which you are applying.
  • Two years of counseling and therapy experience.
  • Ability to work as a member of a team with a great attitude.
  • Demonstrated knowledge of Mental Health diagnoses and therapeutic approaches.
  • Experience collaborating with interdisciplinary teams, including other MPAC providers.
  • Ability to take initiative and effectively communicate in multiple settings with multiple parties (patient, patient’s family, nursing staff, facility social services team, other MPAC providers, etc).
  • High level of organization to manage patient needs, document effectively and ensure the mental health needs of residents are being met.
  • Comfortable with the ever-changing environment of a skilled nursing facility.
  • Knowledge of the Medicare Benefit and commercial insurance plans.
  • Experience in the healthcare setting.

Responsibilities

  • Function as an on-site clinical provider working with residents and their families.
  • Act as a lead member on an interdisciplinary team of physicians and nurse practitioners addressing residents’ emotional, social, and psychological needs.
  • Provide initial assessments.
  • Utilize psychotherapy to address mental health issues.
  • Offer ongoing support to residents.
  • Be responsible for all documentation requirements.
  • Keep up-to-date on Medicare Benefit and commercial insurance plans.
  • Manage caseload and schedule.
  • Foster open communication with the facility team and Regional Clinical Manager.
