About The Position

MPAC Healthcare is seeking Licensed Clinicians (LPC, LMFT, LCSW) to provide high-quality care to seniors. This role offers an autonomous work environment, allowing clinicians to function similarly to a private practice without the administrative burden. The position focuses on clinical work, patient documentation, and facility correspondence, with the opportunity to be an on-site provider working with residents and their families. The clinician will be an active member of an interdisciplinary team, addressing residents’ emotional, social, and psychological needs. This role offers control over caseload, schedule, and salary, with responsibilities including initial assessments, psychotherapy, ongoing support, and documentation in compliance with Medicare and commercial insurance plans.

Requirements

• Master's degree
• Valid LCSW/LMFT/LPC license in the state for which you are applying
• Two years of counseling and therapy experience
• Ability to work as a member of a team with a great attitude
• Demonstrated knowledge of Mental Health diagnoses and therapeutic approaches
• Experience collaborating with interdisciplinary teams, including other MPAC providers, to ensure patients are getting the best care possible
• The ability to foster open communication with the facility team and your Regional Clinical Manager to ensure the success of clinical programs
• Ability to take initiative and effectively communicate in multiple settings with multiple parties (patient, patient’s family, nursing staff, facility social services team, other MPAC providers, etc).
• High level of organization to manage patient needs, document effectively and ensure the mental health needs of residents are being met
• Comfortable with the ever-changing environment of a skilled nursing facility

• Understanding of documentation (CMS, Medicare, Medicaid)
• Nursing home/ hospital facility experience

Responsibilities

• Provide initial assessments
• Utilize psychotherapy to address mental health issues
• Offer ongoing support to residents and their families
• Responsible for all documentation requirements
• Keep up-to-date on Medicare Benefit and commercial insurance plans
• Function as an active, lead member on an interdisciplinary team of physicians and nurse practitioners addressing residents’ emotional, social, and psychological needs.
